Wyoming Healthcare Professional Loan Repayment Program

Loan Repayment for Physicians, Dentists and Allied Health Providers practicing in underserved areas in Wyoming

Program Purpose

The intent of the program is to enhance the ability of medically underserved Wyoming communities to recruit and retain healthcare professionals.


Program Administration

The program is administered by the Office of Rural Health, Rural and Frontier Health Division, Wyoming Department of Health.  It is authorized and funded by the State of Wyoming.  Program requirements are established by W.S. 9-2-118 and 9-2-119 (See Statutes).  The program Rules are promulgated through the Wyoming Department of Health (See Rules).

Wolf 109(a) This is for those individuals notified they were accepted into the Program.  Program awardees must complete and return the original to the Office of Rural Health to recieve payments.

Hints to completing the form:  Typing the document reduces the risk of your information being mis-read.  You can not save what you type, so complete the form, print it out, sign it, and mail it to the Office of Rural Health address below.  You were awarded as an individual, so you will need to check the Individual/Sole proprietor box.  Use the address that is on the contract sent to you with your award notification letter.  If your address has changed, use the new address.  If you want your payments to be electronically made to your bank account, include a voided check with the form when you send it to the Office of Rural Health.  If you have questions, contact one of the staff listed below.

Benefits and Conditions

The program repays qualifying applicants for costs of eligible educational loans in return for a commitment to practice in an underserved area in Wyoming for a minimum of three years.

The current funding formula is:

  • Up to $30,000 each year for three (3) years for a total up to $90,000 for physicians and dentists. 
  • Up to $10,000 each year for three (3) years for a total up to $30,000 for allied healthcare professionals.  As of July 1, 2006, there is no longer a cash match requirement for allied health providers.
